For our new patients, we have conveniently made our patient information forms available to you online and we encourage you to fill them out prior to your appointment. This will save you a considerable amount of time in the reception area. Simply click on a form title to download and print. If you are a former patient, and you believe there have been changes to your medical history or personal contact information, please feel free to bring an updated form with you to your appointment, or contact us by phone or email to make any changes.
What to Bring With You
Please bring the following items with you to your appointment:
- Current medical and vision insurance card
- Current and/or old glasses
- If you wear contact lenses, bring in your contact lens box that has contact lens information for each eye. Please bring your back-up glasses as well.
